Matter of Balance
A Matter of Balance provides practical strategies designed to help reduce the fear of falling and increase the activity levels of older adults who have concerns about falling. In a series of eight two-hour sessions, Med Net One coaches help participants to:

  • view falls and fear of falling as controllable issues
  • set realistic goals for increasing activity
  • make changes in their home environment to reduce risk factors for falling
  • promote exercise to increase strength and balance


The Community Care Travel Team
The Community Care Travel Team (CCTT), a complement to the Patient Centered Medical Home, is a multi-disciplinary health care team (P.A., N.P., R.N. , pharmacist, CDE, RD, behavioral health specialist and exercise specialist) that brings its combined expertise to patients at a physician’s office, the worksite, and educational/community centers to evaluate, provide counseling, and help reduce the risk of complications from chronic conditions.

Diabetes Education Program
Diabetes Self-Management Education (DSME) is the key to better diabetes control and a healthy lifestyle. Med Net One offers DSME classes in group and individual settings. We have a multi-disciplinary team of clinicians providing education including certified diabetes educators, registered nurses, registered dietitians, behavioral health specialists and exercise specialists. Our staff is dedicated to helping patients increase their self-management skills and better control their diabetes.

Med Net One offers Diabetes Self-Management Education (DSME) classes in group and individual settings. We have a multi-disciplinary team of clinicians providing education including certified diabetes educators, registered nurses, registered dietitians, behavioral health specialists and exercise specialists. Classes include: diabetes disease process, meal planning, monitoring, medications, physical activity, healthy coping and preventing long-term complications. Personal Action Toward Health – PATH
PATH is Michigan’s name for the Stanford Chronic Disease Self-Management Program.
The PATH program is a workshop developed and tested at the Stanford Patient Education Research Center. More than 1,000 people with chronic health conditions were followed in a randomized, controlled study for up to three years. Participants in the study that took the program compared to those that did not, have self-reported improvements in general health, fatigue, communications with physicians and social/role activities limitations. They also spent fewer days in the hospital and showed a trend of fewer outpatient visits. PATH is facilitated for two and a half hours a week, once a week, for six weeks. Over the six weeks, emphasis is placed on creating personal, practical and achievable goals.

R-Team™
R-Team™ (Reshaping thru Exercise, Attitudes and Meals) aims to ensure that kids and teens, along with their families, receive lifestyle training and education that empowers them to use positive self-management skills to live a healthier life. Led by a team comprised of a registered dietitian, exercise specialist, wellness coach and registered nurse, the R-Team™ program consists of ten 90-minute sessions. Sessions include interactive educational games as well as instruction on incorporating exercise, healthy food choices and positive body image into everyday life.
