What We Offer

Strong relationship with Priority Health, Blue Care Network and Blue Cross Blue Shield of Michigan’s Value Partnership-Physician Group Incentive Program (PGIP), which aims to:
- Improve performance in healthcare delivery
- Provide members direct access to new care programs, reimbursement and bonus incentive programs
- Provide members access to technology such as population registries, e-prescribing tools and software tracking for patients’ admissions, discharge and transfers (ADT)
Training on chronic health programs and community agency partnerships that help primary care practices better serve diverse patient populations
Hands-on direction, guidance and supervision on:
- Implementation and progress of new healthcare and HIT initiatives (i.e. Patient Centered Medical Home)
