The Patient-Centered Medical Home Neighborhood (PCMH-N) is an extension of the PCMH.

In a Patient-Centered Medical Neighborhood, the primary care provider is in charge of all the different people and places that provide healthcare services to his or her patients, including specialists, hospitals, pharmacies, home health, physical therapists, nutritionists and hospice. Med Net One supports the PCMH-N concept and offers services that enable our physician members to work within such a “neighborhood”.
