Invitation: Medical Network One’s First Quarterly In-Person Meeting with PCP and Practice Team on January 26, 2023

To Our Providers and Staff:

As part of an ongoing effort to provide you and your practice team with timely information, we are
inviting you to a quarterly in-person meeting being held on Thursday, January 26, 2023, at Troy
Embassy Suites, 850 Tower Drive, Troy, MI from 8:00am-12noon. Breakfast will be served at 8:00am
and the meeting will begin at 8:30am. Following the meeting you will have the opportunity to meet
with MedNetOne staff to discuss future payer initiatives including quality improvement activities.

Topics to be addressed:

 Payer contracts and enrollment activities (BCBSM, BCN, HAP, Priority Health, Medicaid and
other)
 Credentialing and recredentialing
 Medicare Advantage programs and STARS rating
 Value-based contracting including full risk
 REACH ACO
 Diabetes Prevention Program (DPP) and Diabetes Self-Management Education and Support
Program (DSMES)
 Care team activities
 Community Health Worker Program
 Behavioral health activities
 Educational opportunities
 New services, opportunities, and resources provided by Medical Network One
