HIE: From Concept to Reality: Interoperability for All

Conference Overview

Health Information Exchange or HIE is one of the core elements of care coordination among health care professionals and care team members. HIE has helped community behavioral health organizations adopt new tools and technology to streamline communication and processes to improve healthcare quality and outcomes.

Conference participants will learn about how health disparities are associated with poor health outcomes. The significance of data sharing on improving patient care and outcomes will be addressed. You will hear strategies on how to identify and mitigate Social Determinants of Health in diverse populations within various settings. Today’s content will challenge the participants to develop new strategies for value-based care.

Hear about HIE success stories from various stakeholders throughout the state of Michigan, and how local initiatives continue to shape communities.

Gain insights into advanced healthcare information exchange and how it has been adopted by various community behavioral health organizations, physician organizations, and primary care practices.
