Pediatric Integrated Care Behavioral Health Clinician

Job Description

Seeking behavioral health clinicians to provide evidence-based consultation, assessments and brief interventions within family medicine primary care practices. In addition, settings include pediatric practices where expertise in ADHD, adolescent depression, and behavioral disorders is required. Experience in team-based care within medical settings is desired. Responsibilities also include assistance in quality improvement projects and population health initiatives.

This is a full-time position. Immediate openings in Downriver, Trenton/Woodhaven area.

Essential Functions

Individual in this position:

  • Provides brief behavioral health interventions and self-management support to patients who present with a myriad of mental health, substance use and medical problems.
  • Provides behavioral health assessments to clarify diagnoses and determine treatment needs (integrating information from clinical interview, medical records, collaborative sources, screening tools and assessment instruments)
  • Manages case load and schedules to optimize efficiency, revenue and sustainability (includes complying with documentation and billing requirements such as obtaining prior authorization as needed)
  • Develops and implements a treatment plan in collaboration with the patient, caregiver, physician and/or other appropriate health care professionals to address the patient’s needs.
  • Performs ongoing monitoring of the plan of care to evaluate effectiveness.
  • Documents care plan progress in Medical Network One’s information system.
  • Evaluates effectiveness of the care plan and modifies as appropriate to reach optimal outcomes.
  • Measures the effectiveness of interventions
  • Engages in population health and quality improvement initiatives.
  • Collaborates and facilitates linkages with community providers and resources
  • Maintains productivity and quality measures as outlined by Medical Network One.
  • Manages and completes assigned work plan objectives and projects in a timely manner.
  • Demonstrates dependability and reliability.
  • Maintains effective team member relations.
  • Adheres to all documentation guidelines.
  • Attends regular staff meetings.
  • Participates in Interdisciplinary/inter-professional care team meetings and professional development activities.
  • Assists orientation and mentoring of new team members as appropriate.
  • Maintains professional relationships with health care provider community and internal and external customers.
  • Conducts self in a professional and ethical manner at all times.
  • Maintains cooperative and effective workplace relationships and adheres to the Medical Network One Code of Conduct.
  • Participates in appropriate continuing educational activities to enhance skills/abilities and promote professional growth.
  • Complies with required safety standards.

Knowledge/Skill/Abilities

  • Strong clinical skills with ability to accurately assess and diagnose mental health and substance abuse conditions.
  • Skilled at implementing evidence-based brief psychological and behavioral interventions including cognitive behavioral therapy, parent training, motivational interviewing and self-management support of psychological and medical conditions.
  • Advanced understanding of interplay between psychological and medical conditions.
  • Demonstrates ability to communicate, problem solve, and work effectively with people.
  • Cultural sensitivity and competence.
  • Excellent organizational skill with the ability to manage multiple priorities.
  • Work independently and handle multiple projects simultaneously.
  • Strong analytical skills.
  • Knowledge of applicable state, and federal regulations.
  • Knowledge of ICD-10 CPT coding, and DSM 5 diagnostic coding.
  • Familiarity with NCQA standards, state/federal regulations and measurement techniques.
  • Ability to take initiative and see tasks to completion.
  • Computer skills and experience with the full suite of Microsoft Office Products.
  • Excellent verbal and written communication skills.
  • Ability to abide by Medical Network One policies.
  • Able to maintain regular attendance based upon agreed schedule.
  • Maintain confidentiality and comply with Health Insurance Portability and Accountability Act (HIPAA).
  • Skilled at establishing and maintaining positive and effective work relationships with coworkers, clients, members, providers and customers.

Required Education

Masters or doctoral degree in Social Work, Counseling, or Psychology.

Required Experience

Three to five years of clinical experience.

Preferred Experience

More than five years of clinical experience. Experience working with people who have comorbid general medical conditions. Skill using motivational interviewing a plus. Experience working on interdisciplinary teams using biopsychosocial model.

Preferred Licensure/Certification

Licensed Clinical Social Worker LCSW, Licensed Professional Counselor LPC, Licensed Marriage and Family Therapist LMFT, Limited Licensed Psychologist LLP, or fully Licensed Psychologist LP in good standing.
