What it an Organized System of Care?
Similar to an accountable care organization, an OSC such as MichCare is a community of clinically trained caregivers that has responsibility for a specific patient population.

Patient Centered Medical Home (PCMH)/ Patient Centered Medical Home Neighborhood (PCMH-N)
The Patient-Centered Medical Home (PCMH) is a national health care movement that establishes the primary care physician’s office as the patient’s medical home. PCMH is founded on the patient/physician relationship, where the primary care physician’s team coordinates all of a patient’s medical care tol improve patient health and outcomes.

Physician Group Incentive Program (PGIP) focuses on improving health care systems across Michigan including:

  • 40 participating physician organizations
  • 5,700+ participating practice units
  • ~20,000 participating practitioners 
  • Over 5,400 PCPs and over 14,500 specialists 
  • PGIP practitioners care for approximately 2.9 million patients across Michigan 

Patient-Centered Medical Home (PCMH) has: 

  • 1,696 practice units that are PCMH designated
  • 4,644 PCPs are PCMH designated

2020 PCMH Performance, for PCMH-designated practices compared to non-PCMH designated PGIP practices (adults 18-64):

  • 26.6% lower rate of emergency department visits
  • 35.5% lower rate of ambulatory care-sensitive inpatient discharges
  • 9.6% lower rate of high-tech radiology usage
  • 5.4% lower rate of low-tech radiology usage
  • 33.1% lower rate of primary care-sensitive emergency department visits 
  • 29.4% lower rate of readmissions 

2020 PCMH Performance, for PCMH-designated practices compared to non-PCMH designated PGIP practices (children 0-17):

  • 29% lower rate of emergency department visits
  • 7.4% lower rate of low-tech radiology usage
  • 39.1% lower rate of primary care-sensitive emergency department visits
